COSMETIC CONSULTATIONS: Please note that there is a nonrefundable $150 consultation fee for an in-person visit, or $120 consultation fee for a virtual visit. This consultation fee will be applied towards any surgeries booked. In order to book a virtual cosmetic consultation, please call our office directly at (947) 217-3223.
FUNCTIONAL/MEDICAL CONSULTATION: Please check here to ensure that our practice is in network with your insurance PRIOR to scheduling. Please note that we are not in network with Medicaid and will not be able to accept Medicaid as primary or secondary insurance. We also recommend you call your insurance to confirm coverage prior to scheduling. If you have questions, please contact us, and our staff would be more than happy to help with the scheduling.
